Clinical measurement and disease course · BEGINNER LESSON

Disease onset

Disease onset is the time when the first symptom or sign is first noticed, or when a study defines the start point for a disease event. In some diseases, this measured start can differ from the underlying biological process that began earlier.

What disease onset is and what gets measured

Disease onset is a recorded starting point used in studies and clinical descriptions of disease course. In amyotrophic lateral sclerosis (ALS), one study defines symptom onset as the date when limb weakness, speech or swallowing difficulty, breathing difficulty, or generalized weakness was first noticed. It also records the site where weakness began, such as limb, bulbar (speech and swallowing), or respiratory, and may record a separate first-symptom date if weakness was not the first symptom. In multiple organ dysfunction syndrome (MODS), onset can also be described by when organ dysfunction appears, either at the same time or one organ after another. In practice, onset is a measured event, not just a general idea.

How onset relates to mechanisms

Onset does not always mean the disease process started at that same moment. In Alzheimer’s disease (AD), one source notes that pathology can begin years before symptom onset, meaning biological changes may be present long before a person notices symptoms. In MODS, onset is linked to mechanisms such as dysregulated inflammation, damage-associated molecular patterns (DAMPs; molecules released from injured or dying cells), pathogen-associated molecular patterns (PAMPs; molecules from microbes), coagulation changes, cell death, and microcirculatory disturbance. The source also describes a ‘first hit’ that starts the inflammatory dysregulation and a ‘second hit’ that can worsen it later. This shows that onset can be tied to the first observable clinical change while the underlying mechanism may already be active or may progress in stages.

Why measuring onset is hard

The sources emphasize that onset definitions vary across studies and can be inconsistent. ALS has major individual differences in symptom onset and progression, so a standardized definition is important for comparing data. Case definitions used to decide who counts as having ALS can also change the sample, and newer criteria may identify more cases than older ones. MODS literature also shows timing differences, including early-onset and late-onset patterns, which depend on the trigger and timing of complications such as infection. In short, onset is useful for tracking disease, but it depends on how the study defines the first event and on how completely early symptoms are captured.

Simple way to think about it

If a person first notices swallowing problems, that can be recorded as onset even if the disease started biologically earlier. If a study wants to compare many patients, it must choose one clear rule for what counts as onset and apply it consistently. This makes onset a measurement tool for disease timelines, not proof that all disease activity begins at that exact moment.

SEE THE IDEA

Disease onset as a measured starting point

Biological changes can start first, the first noticed symptom is recorded as onset, and a study definition links that event to the disease timeline.

Onset is the measured starting point, while the underlying process may begin earlier or evolve in stages.

01 In an ALS study, a participant first notices trouble swallowing on March 3, and weakness begins later. What is the symptom onset date used in the study rule described in the sources?
02 Which statement best matches the source description of AD and onset?
03 A MODS patient has a severe initial injury, then later develops infection that worsens inflammation. Which idea from the sources fits this pattern best?
