SKAT-O
SKAT-O stands for Optimal Sequence Kernel Association Test. It is a gene-based statistical test that combines a SKAT-style component and a burden-style component to check whether a set of variants is associated with a trait.
What SKAT-O is and what it measures
SKAT-O is used in genetic association testing. It asks whether multiple variants grouped together, often within a gene, are associated with a trait. The input is based on score statistics, which are values computed from a regression model, and the covariance among the included variants. The test produces a p-value, which is the probability of seeing a result at least this extreme if there were no true association.

How the method works
SKAT-O combines two ideas: SKAT, which looks for combined evidence across variants, and burden testing, which looks for a shared direction of effect. The test statistic is written as a mixture: Q_SKAT-O = (1 - rho)Q_SKAT + rhoQ_WST, where rho controls how much weight is given to each part. Q_SKAT itself is built from weighted score statistics for the variants. In the cited source, p-values are computed across several rho values, and then combined using ACAT, the aggregated Cauchy association test, rather than simply choosing the smallest p-value.

Why this design is useful
Different genes can show different patterns of genetic effects. Some sets of variants may act in similar directions, which favors a burden-like test, while others may have mixed directions, which favors SKAT-like behavior. By blending the two, SKAT-O is intended to adapt to both patterns instead of assuming only one.

Limitations and caveats
The method depends on the quality of the score statistics, the covariance information, the variant weights, and the chosen rho values. The source also shows that SKAT-O is treated as a mixture chi-square statistic and that p-values are evaluated over a grid of rho values, so the final result is still a statistical approximation rather than a direct biological measurement. In rare-variant analyses, the cited table reports SKAT-O results that were not significant after false discovery rate correction, showing that a nominal p-value does not automatically mean a robust finding.

SEE THE IDEA
How SKAT-O combines two association ideas
Regression-based scores are computed for each included variant.
Captures mixed or differing variant effects across a set.
Captures a common directional effect across variants.
SKAT-O blends the two with rho and reports a p-value.
Scores from variants feed into SKAT and burden components, and SKAT-O combines them into one association test.
SKAT-O mixes two test styles so one analysis can adapt to different patterns of variant effects.
