Cancer immunology / predictive gene-expression model · BEGINNER LESSON
TIDE
TIDE is the Tumor Immune Dysfunction and Exclusion algorithm, a transcriptome-based model that uses gene-expression data to estimate how likely a tumor is to respond to immune checkpoint blockade (ICB). It does this by scoring two patterns: T-cell dysfunction and T-cell exclusion.
What it is and what it measures
TIDE stands for Tumor Immune Dysfunction and Exclusion. It measures patterns in transcriptomic data, meaning RNA-based gene-expression data from a tumor sample. Its output is a probabilistic estimate of response to immune checkpoint blockade (ICB), not a direct measurement of tumor kill or a diagnosis. [6]

How it works
The model looks for two immune-evasion mechanisms. T-cell dysfunction (TCD) describes tumors where T cells are present but have lost cytotoxic ability. T-cell exclusion (TCE) describes tumors where T cells fail to enter the tumor microenvironment, which is the local environment around the tumor cells. TIDE compares a tumor’s expression profile with gene signatures linked to exhausted T cells and immunosuppressive cell types, and it estimates cytotoxic CD8+ T-cell activity using core CTL-associated genes such as CD8A, CD8B, GZMA, GZMB, and PRF1. How results are interpreted
In the excerpted study, tumors were treated as immunologically hot when at least two of three cohort-based criteria were above the cohort mean: IFNG expression, CTL abundance, and low MDSC levels. MDSC means myeloid-derived suppressor cells, a cell type that can dampen immune activity. The text says tumors are predicted to respond to ICB if they are hot with no evidence of TCD, or cold with no evidence of TCE. [0]
Limitations
The method was trained and validated on non-small cell lung cancer and melanoma samples, so its reliability can be weaker in other cancers. It uses bulk RNA sequencing, which averages signals across many cells and cannot separate individual cell types as well as single-cell RNA sequencing. It also gives relative, cohort-dependent scores, so thresholds are not absolute biological cutoffs. Because transcriptomic data is a single snapshot, it cannot show how tumor-immune interactions change over time. [0,2]

How TIDE turns gene expression into a response estimate
Bulk transcriptomic data from the sample
T cells are present but less effective
T cells fail to infiltrate the tumor
Probabilistic prediction for immune checkpoint blockade
TIDE reads the RNA profile, checks for dysfunction or exclusion, and then outputs a likelihood of response to ICB.
