Cilia dysfunction
Cilia dysfunction means a problem in how cilia are built, maintained, moved, or disassembled. It can weaken ciliary signaling, fluid movement, or both, and may lead to disorders called ciliopathies.
What cilia are and what dysfunction means
Cilia are tiny hair-like structures on the cell surface. Primary cilia are nonmoving and act as signaling hubs that sense chemical and mechanical cues, while motile cilia use motion to move fluid across surfaces. Cilia dysfunction means one or more of these jobs fails because the cilium is missing, built incorrectly, cannot move correctly, or is removed at the wrong time.

How cilia work
Cilia depend on their internal skeleton, the axoneme, and on intraflagellar transport (IFT), a two-way shipping system inside the cilium. Anterograde transport moves material from the base to the tip, and retrograde transport brings material back to the cell body. In motile cilia, dynein motor proteins use adenosine triphosphate (ATP), the cell’s energy molecule, to generate beating. In primary cilia, signaling depends on the cilium staying intact and connected to the cell’s control systems.

What goes wrong in dysfunction
If primary cilia are structurally impaired or functionally weak, signaling can change in ways that affect development, tissue organization, and organ function. In kidneys, ciliary signaling defects are linked to cyst formation, abnormal cell growth, and loss of polarity, which means cells lose their normal orientation. If motile cilia beat poorly, people can develop laterality defects, respiratory infections, infertility, or hydrocephalus. Disassembly defects are a separate problem: cilia can be removed too fast or too slowly, changing whether a cell stays ciliated or becomes non-ciliated.

Limitations and disease classification
Cilia dysfunction does not always fit one disease label. Some disorders come from defects in cilia themselves, while others come from non-ciliary proteins that still affect cilia. Also, problems in cilia disassembly may cause disease patterns that do not match classic ciliopathies. Evidence can be incomplete, because some studies are based on single families, limited samples, or models that may not capture every human feature.

SEE THE IDEA
From healthy cilia to dysfunction
Axoneme, membrane, base, and transport machinery are organized correctly.
Primary cilia signal; motile cilia beat; IFT delivers and returns cargo.
Build, transport, signaling, beating, or disassembly fails.
Cells and tissues develop abnormal signaling, movement, or organization.
Proper ciliary structure supports function; structural, transport, or disassembly defects lead to dysfunction and disease.
A cilium can fail at several linked steps, and each step can change cell signaling or motility differently.
